A general health check are available as follows:

NHS Health check

• aged 40-74 (no other conditions, every 5 years)
• aged 75 and over (who has not seen a GP in the last year)

General checks will be carried out as follows:

Blood pressure, blood tests (as appropriate), Height and Weight, Lifestyle advice.

Our team of experienced nurses can carry out essential chronic disease reviews for your long term conditions. These can include:

• Diabetes
• Heart Disease
• Stroke Monitoring
• Asthma
• Chronic Obstructive Pulmonary Disease
• Peripheral Arterial Disease

The reviews include necessary blood tests followed by either a telephone or face to face appointment.
